LEAF Program Registration Form

This form is for students who wish to register for the Laurier English and Academic Foundation (LEAF) program.

The LEAF Program is collecting your information to determine your start date and what level of the LEAF Program you will start in. Your personal information will be kept on file for as long as you are in the LEAF Program, but we will not share this information with anyone.
